Completing the Parents' Financial Statement (PFS) is just one piece of the application process for most schools. In addition to submitting the application, it is imperative that you contact the school(s) directly to make sure you abide by their full guidelines and deadlines. Otherwise, you may neglect to complete a step or two that will jeopardize or delay your financial aid decision.
While some forms will be school-specific, this page lists SSS forms you may be required to submit and tools you might find helpful when completing the PFS. If you have specific questions about completing these forms, call the SSS toll-free helpline at (866) 387-2601.

Supplemental Financial Statement
This financial aid statement should be completed by owners of businesses and/or farms applying for financial assistance. This form is not required of all families; check with the school to verify individual requirements. The form should be returned directly to the school(s) where you are applying for aid. Do not mail this form to SSS.

Designed as a companion piece to the Parents' Financial Statement, this Spanish-language worksheet is available for Spanish-speaking applicants. Families completing this form should transcribe the responses to the English version for submission to processing. PFS Additional School Request Form (ASR) 2007-08
If you have already submitted your PFS for processing, use this form to have the results sent to additional schools rather than completing a new PFS. You can also use this form to request a family report, if you have not already. Mail the completed ASR to SSS at the address provided on the form.

Fee Waiver Program
NAIS recognizes that the PFS application fee itself may present a barrier for some families wishing to apply for financial aid. PFS Fee Waivers are available to families who cannot afford the cost of applying to multiple schools. If you need a fee waiver, please contact one of the schools where you are applying to submit the waiver with your completed PFS. NAIS also approves certain organizations and agencies as fee waiver distributors.
